The Create a Request page stores information about your work request. Use the controls on the page to enter work request information and submit it to Maintenance Management.
To open the Create Request page, click the Create Request button on the Work Request page.

User Name |
Required field. Name given to the account |
Requester |
Name of the individual, or company, requesting the work. |
Required field. Your email address. The Requester Email is used to check work request status.
NOTE: If the feature is enabled, your Requester Email is used as the return address for submitted work request email notifications. |

Phone |
Contact phone number for the individual or company requesting the work. |
Property |
Required field. Primary work location information. This is the primary location of the asset that needs to be maintained. To enter or change the property name in the field, select one from the Property pick list. The pick list contains the names of existing properties. |
Building |
Buildings are the primary location components of a Property, used to further define the location of a property’s assets. |
Asset |
Required field. Name of the area, equipment, or vehicle asset that needs to be maintained. Work is performed on assets. To enter or change the asset name in the field, select one from the Asset pick list. The pick list contains the names of existing assets assigned to the selected Property.
NOTE: If the feature is configured, the names contained in the Asset field pick list are limited to a selected type of asset (area, equipment, or vehicle). |
Location |
Detailed location of the asset assigned to the work request (e.g., physical address). This information is entered in the Location field of the Maintenance Management Work Order form when you send your work request. |
Service |
Required field. Brief description of the service or work to be done.
NOTE: This field is only visible to the user if it is enabled in the Control Center program. |
Description |
Details or specific instructions for the work request.

Reset |
Clicking this link resets the form, clearing all fields filled by the user. |
Create Request |
Clicking this link submits the request to Maintenance Management and creates a new work order.
NOTE: If the feature is enabled, clicking the Create Request button also sends an email notification to a predefined email address.
NOTE: If the feature is enabled, clicking the Create Request button copies all entered work request information to the Comments field of the new work order. |
